If you or a loved one are experiencing a mental health crisis, please do not contact our office. You desire an immediate response therefore, please call 911 immediately for medical attention. If you are not able to call 911, please head to the nearest emergency room immediately.

Your portal has everything you need to communicate with your provider, request or cancel an appointment, request prescription refills, or access your medical records. If you are having difficulties accessing your portal, please contact our office for further assistance.
